When post-construction indoor air monitoring indicates that there is a potential gas/vapor intrusion (VI) condition, all eyes turn to the engineer that designed the vapor intrusion mitigation system (VIMS). It is therefore incumbent upon the design professional to not only design a well-functioning VIMS but also to prepare and implement a robust quality assurance program, which typically begins with the preparation of a Construction Quality Assurance Plan (CQAP). Even then, the importance of regular field visits to observe the installation of the VIMS components cannot be overstated. This presentation will provide a series of examples from completed projects that illustrate a variety of failures and near-misses, ranging from poor design choices to contractor errors that resulted in or had the potential to result in sub-par VIMS performance. The examples presented not only reinforce the importance of regular field visits by the VIMS designer but also provide a practical framework for CQA field visits
